	Files of Life forms, provided by TRIAD and the Page County Sheriff's Office, are used for emergency medical information. The filled out forms (one for each resident) are stored in a plastic magnetized packet that is to be placed on your refrigerator door. They should be filled out in pencil because of updates to medical information. 911 emergency personnel have been alerted to look for the packets on refrigerator doors. Residents are reminded to have their house numbers prominently displayed.
